Primarily it's when people throw in as many keywords as they can, often repeated, in an ill-judged attempt to rank well for them. Fortunately (for the rest of us) it doesn't work, and just makes the site look cheap and nasty.

True...dirty spammers!

using a keyword twice in a url is about all you'll get away with.

Ex. "A place for all your basketweaving needs | BasketWeavingJunkies.com"
