On Page & Correct Phrase Optimisation

When starting a brand new site, I'd usually chose about four to five very different long tail keyword phrases, then work on the anchor text for them until they rank well for them. I would do this for each page, but then, what do I do after that? I mean, when the site is ranking well for the long tail terms, is it possible to remove those long tail terms and move on to new terms, so the original terms are not on the page but they are still very well targetted with back links.

I normally create my content for my reader using all variations of my targeted keywords in various phrases without truly planning which ones I will be focusing on in the future. I then track where my traffic is coming from through time. I sometimes find that even phrases I had not planned on have better placement and traffic than my initially planned keyword phrases or anticipated longtails.
