    Google's information:
This is an advanced feature and should only be used with caution. If used incorrectly, this feature can potentially harm your site’s performance in Google’s search results. We recommend that you disavow backlinks only if you believe you have a considerable number of spammy, artificial, or low-quality links pointing to your site, and if you are confident that the links are causing issues for you. In most cases, Google can assess which links to trust without additional guidance, so most normal or typical sites will not need to use this tool.

    Disavow backlinks

This is a two-step process. First, you’ll need to download a list of links to your site. Next, you’ll create a file containing only the links you want to disavow, and upload this to Google.
    Download links to your site

    1. On the Webmaster Tools home page, click the site you want.
    2. On the Dashboard, click Traffic, and then click Links to Your Site.
    3. Click Download.

You’ll download a file containing all the pages linking to your site. Use this to create a text file containing only the links you want to disavow—one link per line, like this:

spam.example.com/stuff/content.htm If you want Google to ignore all links from an entire domain (like example.com), add the line domain: example.com. Do not upload the entire list of links to your site: the text file that you upload is the list of links you want Google to ignore.
Upload a list of links to disavow

    1. Go to the disavow links tool page.
    2. Select your website.
    3. Click Disavow links.
    4. Click Choose file.

It may take some time for Google to process the information you’ve uploaded. In particular, this information will be incorporated into our index as we recrawl the web and reprocess the pages that we see, which can take a number of weeks.

Also bear in mind google only show you a sample of the links its found, you may require other tools to get a better list of links.
You will be very lucky to get a penalty lifted on your first attempt, It recently took me 10 months posting up a new updated file once a month before it was lifted. This included so many emails to web masters asking for links to be removed before placing them in the disavow file.

First put all you low quality link in txt file and upload it to disavow tool. After that start manual deletion of low quality links. by sending personal mail low quality website webmasters.

This is a pretty sweet tool: Disavow Google Backlinks Check for Disavow link Analysis It basically helps create your disavow file for you by listing live links, dead links, links that Google thinks you have but don't exist and a bunch of other info. It's been awhile since I've used it, but I think it even will spit out a ready-made disavow doc that you can upload directly.

Order the links by domain rather than date so you can identify if you've got domains there with lots of poor links to your site. Start with those and disavow the whole domain, not the individual pages.

As other users have pointed out, you definitely want to try and remove the links first. Unfortunately, Google doesn't want us to just simply disavow the links. We have to show that we put an effort towards removing the links and then disavowing them.
