Getting linked out !
I am building my chiropractic office's website. I noticed from a trial seomoz campaign that my competitor, a small mom and pop chiropractic office, is sporting 4000 + external links and is at the top of serp! It seems obvious that this office has somehow figured out how to game the external links game since know one outside our town would have heard of this office. I mean, it's a small chiropractor office and there should be no frank reason for soo many external links. Is there anything I can do to be competitive with external links?

Quality trump quantity!
I have some medical/health oriented domains and have no problem seeing what is possible for you.
The unfortunate thing here is that quality implies high costs to do things which normally translates to national scaled campaigns simply because there generally isn't enough revenue to be made in one local reach for customers/patients (e.g. for you to make return off the high cost of quality or for me to be worth my while to eat your high costs for an affordable price).
But there are many ways to offset the costs.

there must be something on which the above site is lacking.. use any software like "marketsumarai" so u can see there your competitor..
fill those thing on which your competitor is lacking then compete on those in which competitor are good..
using this method .. sometime even if u don't compete in some like backlinking ( one or two) u can cross him..
