Can I use these 3 tags
using different keywords in each, for 1 image file, without being considered spam?

No it won't be considered as spam. but for the title tag to be more effective in Google the image has to be a link "(u can link it to an anchor). For more ideas check out http://www.seochat.com/viewtopic.php?t=1475.
