Importantly, 301-redirect the original URL to the new URL.
And be patient: pages can take up to a month to (re)index, and in some cases Google can decide isn't important enough to index at all. Good links (including links from trusted sites, from Google+, from Facebook's public pages, and from important pages of your own site) can help with this.
You can also ping the page with pingdevice.com -- it may or may not help speed up things. Ping both the new and the old URL once you have put the 301 redirect in place.
(For more detailed information, on Google see the sticky in this forum.)
