Crawl rates diminish to pages that were once trusted and due to TOS violations are no longer trusted.

That may not be easy to see as it is rare to find a complete website (all pages) untrusted.

You can realize from the drop in traffic and Google will send you signals through webmaster tool. If your site had enough traffic and suddenly it dropped then it means something went wrong with your site.

It depends on the type of penalty imposed by Google:

    1. For manual penalties: It does inform the webmaster of the website.
    2. For algorithmic penalties: It doesn't. One can only track that by analyzing their traffic in Google analytics (or any other analytics tool) and Google webmaster tools.

Yes Google will inform you via Google webmaster tool. Search Engine also send you massage into webmaster tool and you can also check into manual action.

Just check the traffic and impression on the date when any particular algorithmic update is rolled out by Google. If you see any major change, you would likely to get hit by these updates. You need to be very good at interpreting /correlating the both the events. Also you need to have better knowledge of your domain history.

For me, number one would be traffic. If I ever notice a sudden massive drop in traffic, there's got to be something. In the Google webmaster tool, they can send a warnings about your site or activity. There's also an option there to know if Google caught any manual spam action. Check out the metrics in the Google webmaster tool - search queries, impression, clicks, crawls, if they are declining, that should give you the notice.

No, there will be no warning. Just one day, suddenly, the traffic that goes to your site becomes reduced significantly and you won't find your website anywhere in the searchpage. It's very scary when Google is angry, so it will be better if you don't do anything that angers Google.
