Well they can't 'prevent' - they can only proactively address this after-the-fact.

They are actually quite good at this - IP+# of Clicks+TIME INTERVAL= Fraudulent = No Charge.
The bigger problem is network armies = 1,000s of workstations all over the world -- more difficult to address 'early'.

Well yes - if you are advertising elsewhere... but regional campaign setup prevents this.

Overall FRAUD by competitors is a small problem 'most of the time' and a fact of doing business.
To protect yourself - you need to be intimate with your campaigns, run multiple reports daily, weekly, monthly - looking for patterns.
Natural click throughs are "random" and when a pattern can be detected you need to knuckle down, analyze and then show Google your findings.
